Overview

We are seeking a dedicated and compassionate Practice Nurse to join our dynamic team within a General Practice (GP) setting. The successful candidate will play a vital role in delivering high-quality, patient-centered care across a wide range of clinical responsibilities. This position involves working closely with GPs, healthcare assistants, and other members of the multidisciplinary team to support health promotion, disease prevention, and chronic disease management.

As a Practice Nurse, you will be responsible for providing skilled nursing care and clinical services such as immunisations, wound care, cervical screening, and health assessments. You will also support patients with long-term conditions like diabetes, asthma, and hypertension through monitoring and education. The role requires a high level of autonomy, strong communication skills, and a commitment to ongoing professional development.

Main duties of the job
1. Chronic Disease Management
  • Monitoring and supporting patients with long-term conditions such as:
  • Diabetes
  • Asthma
  • Chronic obstructive pulmonary disease (COPD)
  • Hypertension
  • Coronary heart disease
  • Conducting reviews and care planning
  • Educating patients on self-management and lifestyle modifications
2. Health Promotion and Disease Prevention
  • Providing advice and education on smoking cessation, weight management, physical activity, alcohol use, and sexual health
  • Running health screening and NHS Health Check clinics
3. Immunisations and Vaccinations
  • Administering routine and travel vaccinations for adults and children
  • Providing seasonal flu and COVID-19 vaccinations
  • Ensuring vaccine storage and cold chain compliance
4. Cervical Screening
  • Performing smear tests in line with national screening programmes
  • Providing advice and support regarding results and follow-up
5. Wound Care and Minor Injuries
  • Assessment and treatment of wounds, ulcers, burns, and skin infections
  • Post-operative care and suture removal
6. Patient Assessments and Health Checks
  • Conducting new patient checks
  • Performing ECGs, blood pressure monitoring, urinalysis, and phlebotomy
7. Administrative and Collaborative Duties
  • Keeping accurate patient records using EMIS
  • Participating in audits, clinical governance, and quality improvement initiatives
  • Liaising with GPs and allied health professionals

We have a small, friendly and welcoming single handed GP surgery with 2 clinical pharmacists, 1 HCA, 4 admin staff, practice manager and assistant manager.

Administrative Duties

  • Maintain accurate and timely clinical records
  • Use clinical systems (e.g. EMIS, SystmOne) for documentation and patient management
  • Participate in audits, QOF (Quality and Outcomes Framework), and data collection
  • Manage stock control and ordering of nursing supplies and vaccines
Team and Governance
  • Attend practice meetings and contribute to clinical decision-making
  • Comply with infection control, safeguarding, and health & safety policies
  • Engage in continuing professional development (CPD) and revalidation
  • Support training and supervision of students or junior staff as required
Person Specification
Qualifications
Essential
  • Registered Nurse (NMC registration)
  • Previous primary care or community nursing experience
  • Cervical screening, immunisation, and chronic disease management training
Desirable
  • Independent prescribing qualification
  • Experience with SystmOne/EMIS
  • Additional qualifications in chronic disease management or women's health
