Weare currently looking for an experienced Practice Nurse to join our team at ChapelStreet Surgery. This is a part timeposition of up to 25 hours per week. Weare looking for someone who is highly motivated, enthusiastic and committed tojoin our team. A special interest orqualification in Asthma/Diabetes/COPD management is desirable but a willingnessto complete any relevant courses will be required.

The practice has a very experienced, stable team, a single handed GP, apart-time Advanced Nurse Practitioner and part-time Practice Nurse, a full-timeHealth Care Assistant, a Practice Manager and Medical Secretary and a team of receptionists/administrators.We are also supported by various ARRS staff provided by the PCN to which webelong.
The practice is rated as 'good' by the CQC, is a high 'QOF' achiever andhas recently received recognition from NHS England for the excellent patientfeedback received in the 2024 national patient survey.
The practice prides itself on providing a 'family feel' both in the waypatients and staff are treated. We are keen to employ a Practice Manager thatwill understand and contribute to this ethos.
Job responsibilities
- Develop, implement, and embed health promotion and wellbeing programs.
- Create and evaluate individual treatment plans for both acute and non‑acute conditions.
- Support the development of individual treatment plans for patients with chronic diseases.
- Identify and manage patients at risk of long‑term conditions, preventing adverse health effects.
- Provide routine nursing care in accordance with clinical‑based evidence, NICE, and the National Standards Framework.
- Prescribe medication within your scope of practice, adhering to guidelines and protocols.
- Review medication for effectiveness and ensure alignment with national guidance.
- Request and provide advice on prescribed or over‑the‑counter medication.
- Support patients in compliance with prescribed medications.
- Request pathology services as necessary and interpret results.
- Refer patients to other services when appropriate.
- Maintain accurate clinical records / personal care plans, using effective SNOMED codes.
- Maintain chronic disease registers.
- Prioritise health issues and intervene appropriately.
- Recognise, assess, and refer patients with mental health and community health needs.
- Implement vaccination programs for adults and children.
- Liaise with external services/agencies to ensure vulnerable patients are supported appropriately.
- Support the clinical team with safeguarding matters, following local and national policies.
- Conduct a comprehensive physical health assessment of patients with complex multiple healthcare needs.
- Interpret results of different assessments to make diagnosis and plan to deliver care.
- Competently make ethical, evidenced decisions and interventions.
- Prescribe and work with individuals to manage their medicines and refer where necessary.
- Work independently or as part of a multi‑disciplinary team.
- Plan and provide skilled and competent care to meet a patients health and social care needs including referring on to other members of the healthcare team where appropriate.
- To lead on Infection Prevention Control and ensure compliance.
Qualifications
- Registered General Nurse RGN with valid NMC registration.
- Proven experience in primary care
- Competence in chronic disease management, e.g., diabetes, asthma, hypertension.
- Experience with vaccinations, cytology, and wound care.
- Experience with Quality and Outcomes Framework QOF targets and audits.
- Skills and Competencies:
- Excellent clinical skills, including history taking and care planning.
- Ability to work independently and within a multidisciplinary team.
- Strong organizational skills with the ability to prioritise tasks.
- Competence in using electronic patient record systems.
