Practice Nurse (Band 6) – Cornwall Health for Homeless

We are seeking a full‑time Practice Nurse to join our team of GPs. The role supports primary care for people experiencing homelessness in Cornwall, working in clinic sites and through outreach across the county.

  • Provide health assessments, chronic disease management, vaccinations, wound care and sexual/reproductive health services.
  • Deliver health education and screening for infectious diseases.
  • Offer lifestyle and wellbeing advice and advocate for patients.
  • Engage patients flexibly, building trust with those facing barriers to care.
  • Coordinate care with multidisciplinary teams, outreach services and social care.
  • Participate in GP‑led clinics in Truro, Camborne, Penzance and outreach clinics countywide.
  • Flag and address health inequalities to improve overall health outcomes.

Working hours are Monday to Friday, with some outreach work possible. Compassion and flexibility are key.
