Job description
Overview

Now is the time to be part of a healthcare transformation. The new NHS 10-Year Plan puts community care at the centre of service delivery—shifting resources closer to where people live and promoting prevention over hospital admission.

As a member of a community-based NHS trust, you’ll be at the forefront of this change. Work in integrated neighbourhood teams, delivering joined-up care alongside GPs, mental health professionals, social workers, and voluntary services. Benefit from new roles, clear career pathways, apprenticeships, and personalised development support. The plan also brings new workforce standards: improved flexible working, mental health support, safe workplaces, and strong protections against discrimination. With greater investment in underserved areas and a focus on reducing health inequalities, your work will have local impact and national importance.

Community trusts are leading the way in digital innovation, using tools like virtual wards to reduce admin and improve patient care. If you’re passionate about making a difference, this is your chance to help deliver a modern, inclusive, and community-focused NHS.

We have both full and part-time roles available for skilled nurses to join our Community Nursing Teams across sites in East Kent. We are currently able to be flexible with hours and location.

As the Practice Development Nurse will promote excellence in clinical practice, providing professional leadership across East or West Kent locality. You will promote excellence in clinical practice, providing a visible presence and strong professional leadership across our community nursing teams. You will foster an environment which values education as part of the philosophy of the practice area.

Responsibilities
  • Coordinate and facilitate educational activities for registered and non-registered Nursing staff within the designated practice areas.
  • Be a clinically credible practitioner able to work autonomously and with a team, teaching and demonstrating technical clinical skills and assessing competencies within clinical practice.
  • Provide a high level of visibility to staff, patients, relatives and carers across bases and in patients\' homes within the locality.
Working patterns

Additional working pattern: part time and full time hours available. Flexible hours and core of the day 9-5.
