Job description
Role Overview

An exciting opportunity has arisen for a Physical Healthcare Manager within the Low Secure and Community Forensic Care Unit. The post holder will be located at the Wells Road Centre, In-patient unit. The post holder will assume a key role within the in-patient multi-disciplinary team to provide expert clinical case management, advice and support to in-patients who may have long term/deteriorating physical health that may result in declining quality of life or potential hospital admission.

The post holder will be able to combine high level assessment contributing to making differential diagnosis and leading to definitive diagnosis to initiate medical treatment in conjunction with medics, refer appropriately and lead the coordination of care working with partner agencies and key stakeholders.

To assist in the development and implementation of appropriate pathways of care for patients with long term physical health conditions.

To work closely with the staff in the Low Secure and Community Forensic Unit and when necessary, provide expertise and assistance to clinical teams within those services.

You will be responsible for providing clinical and managerial leadership to a high standard, promoting best practice and developing Physical Health services to enhance quality and improve patient experience.

To work in collaboration with the mental health team and offer a comprehensive needs-led service to patients currently residing on a ward within the Wells Road Centre who are known to suffer from one or more physical long-term condition, are at risk of a deterioration in their physical health status and at risk of non-elective admission to an acute hospital environment for treatment and stabilisation of their condition.
