Job summary

Seetec Communities seeks a dedicated Support Worker (Peripatetic) to join the CFO Evolution programme on a long-term Fixed Term Contract until December 2028. You will assess needs, deliver tailored interventions, manage risk, and support participants from custody to community reintegration across the South West region.

You will travel regularly, work with peer mentors and volunteers, and connect participants to housing, health, and substance misuse services while adhering to safeguarding

Qualifications

• Willingness and flexibility to travel across the cluster/region and work in both community and custodial settings.

Responsibilities

• Manage a caseload of participants across custody and community settings.
• Complete pre-release work, through the gate support and create tailored action plans.
• Deliver interventions one-to-one and in groups; provide motivational support.
• Record participant data accurately to meet contractual requirements.
• Meet individual and team performance targets.
