Job summary

Compton Care is seeking a Community and Discharge Coordinator to support transfers of care from acute wards and improve patient flow in the Wolverhampton area. The role involves working with NHS and social care partners across settings.

You will identify patients who could benefit from Compton Care, remove discharge barriers, attend discharge discussions, gather accurate information, and help ensure safe transitions of care.

Qualifications

• Experience in health or social care
• Understanding of discharge planning, patient flow or care coordination
• Excellent communication skills
• Excellent organisational skills
• Ability to manage competing priorities
• Confidence working across organisational boundaries
• Compassionate, confident and solution-focused approach

Responsibilities

• Identify patients across acute ward areas who may benefit from Compton Care support
• Remove barriers to discharge and keep patient flow moving
• Work with multidisciplinary teams, attending board rounds or discharge discussions where required
• Gather accurate information about patients
• Escalate delays appropriately
• Support clear communication with patients and those important to them
• Support safe and effective transitions of care
• Build strong relationships with NHS colleagues and wider system partners
• Strengthen partnership working between Compton Care and NHS and social care colleagues
