James Paget University Hospitals NHS Foundation Trust is seeking a Patient Flow Support Worker to join the Patient Flow team. You will help coordinate discharges, transfers and repatriations, liaising with ward teams and external partners to progress patient pathways and address delays.

You will use OPTICA and WardBoard to monitor pathways, support complex discharge needs, and act as a contact for discharge planning and escalation. GCSEs in English and Maths are required.

Aufgaben

• Support the coordination of patient discharges, transfers and repatriations
• Liaise with ward teams, clinicians, therapists, discharge services and external partners to progress patient pathways
• Identify, investigate and escalation delays and barriers to discharge
• Use OPTICA and WardBoard to monitor pathways, including twice-daily reconciliation of discharge plans, delay reasons and estimated discharge dates
• Monitor and coordinate repatriations to local hospitals, specialist units and acute providers
• Support patients with complex discharge needs, including care packages, rehabilitation, continuing healthcare assessments, housing support and specialist transport
• Act as a point of contact for discharge planning and patient flow escalation
• Attend and support patient flow, long length of stay, escalation and discharge meetings, recording and progressing agreed actions
• Coordinate discharge transport and complex transport such as repatriations and bariatric transfers
• Advise and train staff on OPTICA, patient flow processes and transport booking systems
• Promote data quality through coaching and auditing of patient flow information
• Respond to telephone and email enquiries and maintain accurate, confidential patient records
