You will assess capacity, support safeguarding, maintain accurate records, and work with families and carers to plan care in a busy environment. Flexible working and safeguarding training are involved, with opportunities to contribute to audits and service improvements.

Qualifications

• Experience in safeguarding adults and children.
• Ability to work in a busy, changing environment.

Responsabilités

• Support complex discharges from acute hospital to appropriate level of service.
• Take a holistic approach with the person, family or system to enable sustainable outcomes.
• Maintain clear, accurate records of advice, observations and actions.
• Assess, plan, implement and evaluate care, escalating concerns as needed.
• Engage in multiagency collaboration and participate in audits/surveys.
