Job summary

Kent Community Health NHS Foundation Trust is recruiting a Transfer of Care Nurse to support complex discharges from hospital and coordinate with health and social care colleagues. The role focuses on holistic care for patients, families and carers, with safeguarding and multiagency collaboration at its core.

You will document care plans, escalate concerns, and work across community teams to ensure safe, sustainable outcomes for vulnerable individuals in a fast-changing environment.

Qualifications

• Level 6 degree or equivalent in nursing.
• Registration with the Nursing and Midwifery Council.
• Experience managing complex caseloads.
• Mentoring or supporting others.
• Customer-focused approach and IT skills.

Responsibilities

• Assess, plan, implement and evaluate patient care, escalating concerns and responding to changes in condition.
• Support complex discharges from the acute hospital with health and social care colleagues.
• Work holistically with individuals, families and carers to build relationships and networks and achieve sustainable outcomes.
• Recognise and respond to abuse and risk to adults and children, assess capacity and support decision making.
• Work jointly with safeguarding teams, partner organisations and multiagency partners to keep clients safe and reduce risk.
• Maintain clear, accurate records of advice given, observations and action taken.
• Balance changing tasks within the team while working independently and using initiative.
• Follow policies and procedures and take part in audits, surveys and consultation on proposed changes.
