West London NHS Trust is seeking a Transition Support Worker to assist patients moving from acute hospital care back into the community. You will provide practical and emotional support to promote independence and prevent readmission, working with inpatient and discharge teams.

The role offers exposure to services across the Trust and coordination with community organisations to ensure smooth transitions and strong continuity of care for patients with complex needs.

Qualificações

• Excellent people skills required.
• Ability to work with individuals transitioning from acute admission to community settings.

Responsabilidades

• Collaborate with Clinical Lead to support discharge planning and community integration.
• Facilitate smooth transitions from hospital to community settings and identify triggers and coping strategies.
• Link service users with community groups and resources to promote social inclusion and recovery.
• Build respectful, supportive relationships and tailor recovery plans to individual goals and cultural backgrounds.
