Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Patient Flow Coordinator to help ensure timely, safe discharge from acute hospital to community services. You will work with nurses, social workers and community partners to coordinate care and gather information for patient-centred discharge planning.

Join a multidisciplinary team and play a key role in reducing delays, maintaining patient safety, and supporting patients and families through a stressful period.

Formación

• Experience in discharge planning and patient flow processes.
