Responsibilities

• Support delivery of high quality discharge and patient flow for complex planning needs.
• Liaise with ward staff to identify patients needing intervention and coordinate appropriate assessments/referrals.
• Communicate with patients, relatives, carers, community staff and services to develop an optimum discharge plan.
