Bedfordshire Hospitals NHS Foundation Trust is seeking a Senior Assistant Practitioner and Coordinator to support the Transfer of Care Service, hospital discharges, Urgent Care Response and Virtual Wards while maintaining clear communication with partners. The role involves managing a caseload for rehabilitation and self-management, and supporting the service during high pressure periods.

The successful candidate will travel across Bedfordshire with their own vehicle.
