Job description

This is a great opportunity to join a dynamic Primary CareNetwork in Epsom, Surrey. The role offers a mix of 1) home visits with closeliaison with our practices and our community team (matrons, DNs,physiotherapists etc) and 2) practice based role within a large, thrivingpractice in the centre of Epsom.

The Paramedic will work autonomously both within the community and within the PCNand Practice based multi-disciplinary team.

Main duties of the job
  • Assess patientspresenting with acute illness or for review of chronic disease conditions.
  • Undertake homevisits for acute illness or chronic disease management.
  • Assess, diagnosis,plan, implement and evaluate interventions/treatments for patients withcomplex needs and follow up as appropriate.
  • Advise patients ongeneral healthcare and promote self-management where appropriate,including signposting patients to other community or voluntary services.
  • Perform specialisthealth checks and reviews; perform and interpret ECGs; performinvestigatory procedures as required. Undertakethe collection of pathological specimens including intravenous blood samples,swabs etc.
  • Support thedelivery of anticipatory care plans as this process develops (e.g.monitoring blood pressure and diabetes risk of elderly patients living insheltered housing).
  • Provide analternative model to urgent and same day GP home visit.

Epsom PCN brings together six GP practices across the Epsomand Ewell area, serving a population of approximately 63,000 patients. We are awell-established and forward-thinking network, supported by a multidisciplinaryteam of around 25 to 30 additional roles, including Clinical Pharmacists,Paramedics, Physician Associates, First Contact Physiotherapists, PharmacyTechnicians, Care Co-ordinators, GPs, and a dedicated Health & WellbeingTeam.

Job responsibilities

The Paramedic/Practitioner will work autonomously both within the community and within the PCNand Practice based multi-disciplinary team, using their clinicalassessment and treatment skills, to provide first point of contact for patientspresenting with undifferentiated, undiagnosed problems.

CoreDuties:

The Paramedic/Practitionerwill:

  • Assess and triagepatients, including same day triage, and as appropriate provide definitivetreatment (including prescribing medications following policy, patientgroup directives, NICE (national) and local clinical guidelines and localcare pathways) or make necessary referrals to other members of the primarycare team.
  • Assess patientspresenting with acute illness or for review of chronic disease conditions.
  • Undertake homevisits for acute illness or chronic disease management.
  • Diagnose, plan,implement and evaluate treatment/interventions and care for patientspresenting with an undifferentiated diagnosis.
  • Clinically examineand assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly.
  • Assess, diagnosis,plan, implement and evaluate interventions/treatments for patients withcomplex needs and follow up as appropriate.
  • Proactivelyidentify, diagnose and manage treatment plans for patients at risk ofdeveloping a long-term condition (as appropriate).
  • Diagnose andmanage both acute and chronic conditions, integrating both drug- andnon-drug-based treatment methods into a management plan.
  • Advise patients ongeneral healthcare and promote self-management where appropriate,including signposting patients to other community or voluntary services.
  • Perform specialisthealth checks and reviews; perform and interpret ECGs; performinvestigatory procedures as required.

Undertakethe collection of pathological specimens including intravenous blood samples,swabs etc.

  • Support thedelivery of anticipatory care plans as this process develops (e.g.monitoring blood pressure and diabetes risk of elderly patients living insheltered housing).
  • Provide analternative model to urgent and same day GP home visit.
  • Maintain accurateand contemporaneous health records appropriate to the consultation,ensuring accurate completion of all necessary documentation associatedwith patient health care and registration with the practice.

Other Duties:

  • Work within thepolicies of scheme of the practices and PCN.
  • Promote client involvementin the management of the service.
  • Participate inregular supervision sessions and appraisals with your line manager. Attend training and developmentactivities as identified and participate in meetings as required.
  • Maintainadministration systems and workspace used in a clean and tidy condition.
  • Work flexibly tomeet the needs of clients.
  • To undertake anyother duties appropriate to the grade and purpose of the job as may beagreed by the post holder.

Communication:

  • Demonstratesensitive communication styles to ensure patients are fully informed andconsent to treatment.
  • Communicateeffectively with patients and carers recognizing the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating.
  • Anticipatebarriers to communication and take action to improve communication.
  • Maintain goodcommunications with both the doctors and the practice management team, inorder to sustain a high standard of service and safety to the patient
  • The post holderwill promote good communication within the primary care team by beingapproachable and knowledgeable regarding all aspects of patient services.
  • The post holderwill report to the Clinical Director/Managing Director of GPHP to discussrequests for personal annual leave or TOIL and ensure cover of the post.
  • The post holderwill communicate face to face with all team members on a daily basis,utilise telephone and email as a means of internal and externalcommunication and regularly take part in team meetings to ensure updatedknowledge of systems and procedures or other important information isgained and/or shared.

Health & Safety

  • Maintain a goodworking knowledge of Health and Safety procedures and fire precautions,and operate the correct procedures and participate in policy developmentand data collection where appropriate.
  • The post holderhas a duty of care and personal obligation to act to reducehealthcare-associated infections (HCAIs). They must attend mandatory trainingin infection prevention and control (IP&C) and be compliant with allmeasures required by the trust to reduce HCAIs. All post holders mustcomply with the practice infection and immunisation policies as well as befamiliar with the Practices Infection Prevention & Control policies,including those that apply to their duties, such as hand decontamination,personal protective equipment, aseptic techniques and safe disposal ofsharps.
  • Be fully aware ofthe potential health and safety hazards in the practice environment. In the event of an accident, whether astaff member or a patient, immediately notify the Practice Management
  • Generalhousekeeping duties such as cleaning up after spillages to avoid accidentor injury
