What you will do:

  • Manage a caseload of complex situations, dealing with clients facing multiple physical, cognitive, and environmental challenges.
  • Conduct thorough home visits to evaluate functional needs and assess the necessity for adaptations or equipment.
  • Carry out manual handling assessments and create safe handling plans involving hoists, slings, and transfer aids.
  • Prescribe and manage major adaptations such as level access showers, stairlifts, ramps, and through-floor lifts, ensuring collaboration with housing and DFG teams.
  • Provide clinical reasoning for high-cost equipment and necessary structural changes in line with legislation and best practice.
  • Liaise with contractors, surveyors, and housing officers to ensure each adaptation meets client needs safely and effectively.
  • Support hospital discharge planning with timely provision of essential equipment and adaptations to facilitate smooth transitions.
  • Maintain detailed documentation and contribute to legal reports, safeguarding enquiries, and funding panels.
  • Offer supervision and peer support to junior staff, promoting safe manual handling practices and reflective learning.
  • Serve as the primary contact for service users during the adaptation process, offering guidance, advocacy, and support.
  • Collaborate with partner agencies and internal teams to ensure a coordinated approach to service delivery.

You must be registered with HCPC and have a valid driving licence for this role to manage home visits effectively and efficiently.
