Carry out detailed assessments and prescribe appropriate moving and handling equipment to meet client needs.
Deliver single-handed care interventions and complete reviews aimed at reducing reliance on care packages.
Work collaboratively within a multidisciplinary team to support Adult Social Care service users.
Utilise experience—ideally with Liquid Logic—to enhance case management and service delivery.
Maintain accurate, high-quality documentation of all assessments, interactions, and outcomes.
Engage sensitively with clients, carers, and families to promote safe and effective moving and handling practices.
Degree or MSc in Occupational Therapy.
HCPC registration.
Minimum 2 years’ experience within Adult Social Care.
Driving licence preferred for efficient travel between sites.
