Job description

The OT teams work closely with the Physios and the innovative Home First team that undertakes discharge home visits with the care agencies using a Discharge to Assess approach.

Responsibilities
  • To work clinically assessing acute medical in-patients and providing recommendations regarding their ability to be discharged home or requiring some in-patient rehabilitation and liaising with all the community services available.
  • To support your team to do the same and identify and develop your own interests supported and empowered by the Occupational Therapy manager.
  • To work closely with the multidisciplinary team and lead and develop a most effective model and vision with your dynamic team, supervising and delegating.
  • To contribute to interdisciplinary team management of long term disability and its consequent problems by use of specific clinical skills to treat.
  • To be able to identify own professional limitations and seek professional advice when necessary.
  • To undertake responsibility and leadership for completion and implementation of clinical projects, audit or research.
  • To analyse and interpret clinical and non-clinical information to contribute to an accurate treatment approach.
