A leading healthcare provider in Leeds is looking for a motivated Nurse Team Leader for Neonatal Outreach. The transition from an intensive care setting to home is a process that takes careful planning and preparation. As the lead for Neonatal outreach, you will manage the outreach team who will guide and support the process of discharge in the ward setting to the home setting.
This role requires autonomous working therefore extensive Neonatal clinical knowledge is needed to aid assessments and decisions that you are making as well as supporting the team with their caseloads. You will be responsible for the management and overseeing of the outreach caseload, delegating, organising and supporting others workload as well as taking a caseload within this role.

The Neonatal outreach senior sister will provide support lead and manage a team of registered nurses and nursery nurses, ensuring the coverage of a 7-day service.
The postholder will work collaboratively with the multidisciplinary team to deliver care to Neonates transitioning from hospital to home, with the aim of earlier discharge if appropriate.
They will assess, plan and implement and evaluate care for Neonates requiring ongoing support in the community, acting as liaison and advocate between the families and other health professionals and services.
The postholder will serve as a specialist resource for both the Neonatal units and secondary healthcare providers, offering expert guidance and educational needs.
As the team leader you will manage and contribute to service development, training, audit and support quality improvement projects to improve evidence-based care to our patient population.
Independent decision making is integral to this role as a practitioner and team lead, to effectively coordinate the multiple aspects of care therefore excellent communication skills, autonomy and flexibility is essential.
The Leeds Centre of Newborn Care at Leeds Teaching Hospitals Trust is one of the largest in the UK. The service is currently provided across two sites, Leeds General Infirmary and St James's University Hospital. We have a NICU and surgical unit at LGI, SCBU at SJUH, Transitional Care Unit and an Outreach Service.
