Overview

Delivery of a high standard of nursing care to individuals referred to the Memory & Dementia Service. Provide a specialist mental health nursing assessment, care planning and to evaluate those experiencing dementia. To work in partnership with fellow team members and internal/external partners. To undertake their own professional development and compliance with mandatory training.

Role Opportunity

An opportunity has arisen to recruit a band 7 mental health Nurse Practitioner within the Memory Assessment Pathway, based in Lichfield/Burntwood and Mercian PCN. Applicants should possess the knowledge and skills to enable them to work in the memory and dementia speciality, working with service users affected by all types of dementia. This includes working with challenging and complex needs associated to a dementia diagnosis. Applicants must have non-medical prescribing qualification. Applicants will be required to work closely with the existing multi-disciplinary team members, providing diagnostic assessment and follow ups to those referred. The role will include close working with physical and social care colleagues along with our GP and primary care colleagues.
