Job summary

A London-based NHS Healthcare Provider is seeking a Mental Health Care Co-ordinator to support care delivery in a prison setting. Overview

Our NLFT Prison situated in South London has a vacancy for a Mental Health Care Co‑ordinator. The core of this role is to support the delivery of care in the form of new models of care. The post holder will work with staff across healthcare and the wider prison establishment, providing specialist advice to GPs in the assessment and treatment of patients on the prison residential wings.

Key Responsibilities

The successful candidate will be responsible for:

  • Implementing sound clinical skills in mental health practice, including CPA care coordination, group work, risk assessment and crisis intervention.
  • Managing own caseload as directed by the Mental Health Hub Manager.
  • Providing skilled assessments of patients’ emotional and psychological needs based on complex data obtained from the patient and other sources.
  • Delivering psychosocial interventions for patients with moderate to complex mental health difficulties.
  • Jointly facilitating group work with multi‑disciplinary colleagues, including education staff.
  • Completing clinical risk assessments at first and subsequent assessment interviews and accurately recording and reporting key risks.
  • Developing care plans in collaboration with the patient, carer and other relevant person or agency.
Qualifications and Experience

  • Experience in a prison or forensic setting is desirable.
