• Local area knowledge across London is essential.
• Experience in Mental Capacity Assessments, Care Planning, and Risk Assessments is preferred.

Responsibilities

• Work collaboratively with the existing team of care coordinators.
• Respond professionally to issues and risks reported by staff teams.
• Ensure compliance with local authority and CQC requirements.

Skills
