About the Role

Are you proactive and passionate about innovation and providing a high-quality holistic approach to the care of people who need mental health support? This role will allow you to work in an integrated MH setting with colleagues from primary care, mental health services, including the voluntary and community sector, to improve access to support for people with mental health issues.

As part of our services across Oxfordshire we are implementing Keystone Mental Health hubs to support GP practices with Mental Health assessment and support for patients in their communities.

We will be offering training and development to all our Hub staff to ensure they are able to provide a trusted assessor role, as well as being supported to apply for formal training currently being offered from HEE to provide psychological interventions for people with SMI.

If you recognise the longstanding gap between Primary Care and traditional secondary care mental health services for patients and are an experienced community Mental Health nurse or OT and really want to make a difference and close the gap we look forward to receiving your application.

Responsibilities
  • As the lead clinician you will work with a team of mental health professionals, Peer Support Workers and 3rd sector partners to provide the initial mental health intervention for 2-3 PCNs.
  • You will provide triage, assessment and short-term interventions for people presenting for the first time with signs of mental illness, as well as for those with an SMI who are under the care of Primary care, offering early intervention and relapse prevention.
  • You will support the team to focus services on local communities and local GP practices to help rebuild and reconnect healthcare teams across the communities they serve.
