We’re seeking a compassionate and motivated Community RN / Home Care Coordinator to join our Karingal Green Home Care team in Kalamunda.

About the Role

As a key member of our Home Care team, you will help deliver a range of services that support older people to live safely, independently, and comfortably at home. You will work with clients across varying levels of need, from entry‑level support to more complex care requirements.

In this pivotal role, you will:

  • Oversee the implementation and ongoing management of Support at Home services in partnership with our service delivery team and external providers.
  • Act as the main point of contact between Hall & Prior, clients, and their care teams.
  • Ensure care plans are responsive, goal‑oriented, and tailored to each client’s preferences and wellbeing outcomes.
  • Travel within the region to conduct home visits and provide in‑person support.
Key Responsibilities
  • Conduct client assessments and develop person‑centred care plans in consultation with clients and families.
  • Coordinate, monitor, and review services to ensure alignment with client goals, approvals, and funding allocations.
  • Build and maintain positive relationships with clients, families, service providers, and health professionals.
  • Ensure compliance with Aged Care Quality Standards, organisational policies, and relevant legislation.
  • Manage client budgets and communicate package expenditure clearly and transparently.
  • Identify and escalat[e] risks or changes in client condition.
  • Support continuous improvement and contribute to a culture of safety, respect, and excellence.
What You’ll Bring
  • Tertiary qualification in Nursing or Community Services (or equivalent experience)
  • Minimum 3 years’ experience coordinating community‑based aged care (HCP/CHSP/VHC)
  • Strong understanding of My Aged Care systems, including the new Support at Home model and aged care funding
  • Demonstrated case management skills and the ability to work independently
  • Experience working with and adhering to budgets
  • Excellent communication and relationship‑building skills
  • Current driver’s licence and fully insured vehicle
  • Current CPR certificate (or willingness to obtain)
  • NDIS Worker Screening
