An excellent opportunity has arisen for the right candidate to join our Integrated Community Services Business Unit as a Heart Failure Nurse Specialist based at Fartown Health Centre. Our Heart Failure Nurse Specialist team comprises of experienced specialist nurses, staff nurses and health care assistants.
The successful candidate will have an extensive cardiology background and be passionate about providing high standards of care and management for patients diagnosed with chronic heart failure condition. You will work in the community to support patients living in Huddersfield and also be a part of the wider Locala Kirklees Heart Failure Nurse Specialist service.
