Job Overview

Shaston ward is a 15‑bedded ward looking for a Band 3 Healthcare Assistant who is motivated to make a real difference with a caring and compassionate attitude, excellent team‑working, communication and assessment skills. We provide rehabilitation and end‑of‑life care within our thriving community hospital and are looking for assistants with a keen interest in rehabilitation/frailty and end‑of‑life care. The role is part of a forward‑thinking supportive multidisciplinary team.

Responsibilities

• Provide a high‑quality person‑centred approach to care delivery within your sphere of practice always considering patients privacy, dignity and safety.
• To deliver planned care to a group of patients working alongside the multi‑disciplinary team and in accordance with Trust policies and protocols.
• Demonstrate the ability to work effectively as part of a team and provide support/guidance to less experienced staff/learners within the clinical environment as needed.
• Manage a defined group of patients/service users under the indirect supervision of a Registered Nurse, using evidence based/client centred principles to undertake a range of delegated tasks independently.
• Contribute to the ongoing assessment, planning, implementation, and monitoring of patient/service user care, and escalat to senior registered practitioner as appropriate.
• Promote health and prevent ill health in service users, contributing to integrated care.
• To plan and undertake clinical tasks guided by standard operating procedures and protocols, within the limitations of the Healthcare Assistant role.
• Undertake delegated clinical skills within their scope of practice having once undertaken relevant training, deemed competent and adhere to local and national guidance.

Qualifications & Requirements
