Shaston ward is a 15‑bedded ward looking for a Band 3 Healthcare Assistant who is motivated to make a real difference with a caring and compassionate attitude, excellent team‑working, communication and assessment skills. We provide rehabilitation and end‑of‑life care within our thriving community hospital and are looking for assistants with a keen interest in rehabilitation/frailty and end‑of‑life care. The role is part of a forward‑thinking supportive multidisciplinary team.
Responsibilities
- Provide a high‑quality person‑centred approach to care delivery within your sphere of practice always considering patients privacy, dignity and safety.
- To deliver planned care to a group of patients working alongside the multi‑disciplinary team and in accordance with Trust policies and protocols.
- Demonstrate the ability to work effectively as part of a team and provide support/guidance to less experienced staff/learners within the clinical environment as needed.
- Manage a defined group of patients/service users under the indirect supervision of a Registered Nurse, using evidence based/client centred principles to undertake a range of delegated tasks independently.
- Contribute to the ongoing assessment, planning, implementation, and monitoring of patient/service user care, and escalat to senior registered practitioner as appropriate.
- Promote health and prevent ill health in service users, contributing to integrated care.
- To plan and undertake clinical tasks guided by standard operating procedures and protocols, within the limitations of the Healthcare Assistant role.
- Undertake delegated clinical skills within their scope of practice having once undertaken relevant training, deemed competent and adhere to local and national guidance.
Qualifications & Requirements

- Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
- Band 3 Healthcare Assistant level with a keen interest in rehabilitation/frailty and End of Life Care.
- Excellent communication, teamwork, and assessment skills.
