Bridgewatercommunity health community trust are seeking a dedicated and compassionate HealthVisitor to join our dynamic Halton team, working to improve outcomes forchildren, families, and communities. As a qualified Specialist Community PublicHealth Nurse (SCPHN), you will deliver the Healthy Child Programme,providing vital support from pregnancy through to school entry.

You willwork in partnership with parents, carers, early years providers, and otherprofessionals to promote health and wellbeing, identify health needs early,prevent problems from escalating, and safeguard children. Main duties of the job

Halton is a geographically small area however provides a diversecommunity, ranging from affluent tosocially deprived. It is anarea that provides excellent experiences for Health Visitors, especially thosethat wish to specialise in specific areas.

The post holder will beresponsible for assessing the needs of the local population and deliveringhealth promotion services to promote health & well-being and prevent illhealth amongst Children, Young People & Families.

You will work as part of oneof the teams in Halton, consisting of Team Leaders, Health Visitors, CommunityNurses & Nursery Nurses. This is where you will be supported whilst workingwith your own individual caseload, as well as in partnership with the newlydeveloped Family Hubs to drive forward the national agenda.

Job responsibilities
  1. To support delivery of the Healthy Child Programme using evidence based practice to actively promote thehealth of children and young people.
  2. To be a named / link nursefor designated schools/ early years settings whilst holding a caseload whichincorporates the 4 levels of intervention (Community, Universal, Universal Plusand Universal Partnership Plus).
  3. To deliver and engage inhealth promotion programmes and public health initiatives to prevent illhealth, early identification of abnormalities and work with all clients in the0-19 service and communities to reduce health inequalities as identified withinindividual Health Plans.
  4. To conduct health needsassessments in the home, school, clinics or community settings in a plannedprogramme of care within a variety of settings.
  5. Offer appropriateinterventions, delivering agreed programmes of care and evaluating the outcomesfor the child, young person and family.
  6. Plan and implement schoolentry assessments delegating duties as appropriate.
  7. Plan and implement healthassessment at transition from primary to secondary school delegating duties asappropriate.
  8. Undertake comprehensivehealth assessments for identified babies, children and young people, includingannual and bi annual health assessments for those in local authority care inline with the Healthy Child Programme for children 0-19 years of age.
  9. Establish clear andeffective communication links with children and young
  10. people, parents/carers,education staff, and a range of other professionals from both voluntary andstatutory organisations.
  11. Communicate outcomes ofassessment and future care planning to the named SCPHN health visitor or SCPHNschool nurse
  12. Carry out home visits tochildren and families advising on all aspects of health as appropriate.
  13. Participate in health needsassessment of the local community, contributing to the planning, implementationand evaluation of strategies to address unmet health needs.
  14. To identify and assesschildren 0-19 with complex health needs, offering an enhanced integratedservice within the multi-agency arena.
  15. To assess the needs ofchildren and families following attendance at A&E, discharge from hospitalworking within trust guidelines.
  16. To identify, assess, reportand work independently with oversight from the SCPHN School Nurse or SCPHNHealth Visitor to safeguard and protect the health and wellbeing of childrenand young people working within safeguarding policies and guidelines. To attendChild Protection case conferences and other meetings as appropriate.
  17. To provide on-going supportfor children and young people in care including the statutory health assessmentand on-going review.
  18. Listen appropriately andtake into account any sensitive issues and communicate these to appropriatepersons/agencies ensuring that children are protected and that disclosures areacted upon appropriately.
  19. Feedback the views andneeds of children/young people, families and schools to the named SCPHN SchoolNurse/SCPHN Health Visitor as necessary, to enable appropriate interventions tobe implemented.
  20. Ensure efficient andeffective use of resources.
  21. Support the delivery ofnurse led immunisation programmes as service needs, following patient groupdirections and actively promoting the National and Local ImmunisationProgrammes to families.
  22. To work in collaborationwith other agencies to deliver services to children, young people and theirfamilies.
Clinical
  1. Maintain active status onNMC register acting in accordance with the professional code and guidingdocuments in adherence with Revalidation Framework.
  2. Maintain contemporaneous,factual records in accordance with Trust guidelines and policies.
  3. Be aware of current workingpractices in accordance with Trust policies and procedures.
  4. Use their own professionaljudgement to identify solutions to a range of day-to-day problems, seekingadvice where appropriate from the SCPHN
  5. Manage their timeeffectively to ensure prioritisation of workload in order to achieve agreeddeadlines.
  6. Be responsible for ownprofessional updating and keep abreast of current research based practice.
  7. Develop a working knowledgeand experience of IT systems
  8. Observing the complaints anddisciplinary procedures in accordance with the Trust policy.
  9. Contribute to clinicalaudit, research and clinical governance as part of the 0-19 public healthnursing Team.
  10. Participate in and completethe agreed induction programme for newly appointed staff
  11. Manage time with the teamleader and team to ensure service is covered.
  12. Report sickness absence andreturn relevant forms to line manager.
Leadership:
  1. Act to maintain anappropriate learning environment to meet the educational and developmentalneeds of student nurses and other learners as required
  2. Support other nurses andstaff within the team, including the induction of new staff members. Ensure effective learning experience andopportunities to achieve learning outcomes for skill mix team.
  3. Develop and contribute tothe development and implementation of policies, procedures and guidelinesrelevant to own area of work. Provideadvice where services are developing.
Supervision and Development
  1. The post holder will be ableto recognise their own level of professional competence and identify sources ofsupport as required.
  2. Provide mentorship,preceptorship and clinical supervision to staff as appropriate.
  3. Support the development ofthe team by teaching and assessing junior members of the team.
  4. Undertake mandatory andstatutory training that support the post holders role.
  5. Undertake training specificto the post.
  6. Participate in individualperformance reviews (PDR) and clinical supervision.

This job description is anoutline of the role and responsibilities. From time to time, due to the needsof the service, we may ask you to flexibly undertake other duties that areconsistent with your role and banding.

The 0-19 service iscommissioned to deliver a service 8am-8pm and offering weekend delivery,dependant on the needs of the service.

As the 0-19 service isdelivered across Halton you will have a base for administrative purposes only,as we may ask you to deliver a service from a variety of bases dependent onneeds of the service.
