A regional healthcare provider in Hull is seeking Health Visitors to join their innovative team. The role involves managing the health needs of a 0-5 year caseload with a strong focus on care continuity. Candidates are expected to hold current NMC registration and a professional qualification in Specialist Community Public Health Nursing, along with relevant experience in children's services. This is an exciting time for the Hull 0-19 service, with opportunities to influence and innovate as we work towards an ambitious plan to enhance the current service with a new service specification awarded to Humber Teaching NHS Foundation Trust.
We are looking to recruit to a number of Health Visiting posts - which will be based across a variety of locations in Hull. We can offer full or part-time working patterns alongside a supportive preceptorship and orientation programmes.
Applicants are required to be committed to providing the best possible service to children, young people and families. They should demonstrate willingness to develop and maintain integrated services.
The post holder will be responsible for the day-to-day management of a 0-5 year caseload, with an emphasis on continuity of care and support where appropriate, assessing health needs of the local population and care planning to meet need as per the revised Healthy Child Programme - Universal, Targeted and Specialist pathways of care.
