A leading health organization is seeking a HealthPathways Clinical Coordinator to support the development and improvement of a clinical information portal. This role involves coordinating clinical pathways, engaging stakeholders, and contributing to patient care in South Australia. The HealthPathways Clinical Coordinator role supports the development, implementation, and ongoing improvement of HealthPathways across South Australia, an online clinical information and referral portal used at the point of care to support assessment, management, and referral of patients. Working in partnership with Country SA PHN, Adelaide PHN, Wellbeing SA, and Streamliners NZ, the role coordinates the localisation of clinical pathways, supports stakeholder engagement, and contributes to consistent care delivery, improved referral clarity, and a better patient journey across the South Australian health system. The position requires collaboration with internal teams, GP Clinical Editors, health service providers, and external partners to ensure HealthPathways SA remains a trusted, high-quality clinical resource.
