A Primary Care Network in Matlock, UK, is seeking an enthusiastic GP for its Home Visiting Service. This permanent role involves providing patient care through home visits, assessments, and consultations across multiple practices in the Derbyshire Dales. You will need full GMC registration and experience in primary care. The position offers opportunities for varied experiences and professional support in a beautiful location, working closely with a range of healthcare professionals.
Derbyshire Dales PCN is looking to recruit an experienced GP who is enthusiastic & forward-thinking to join our Home Visiting Service on a permanent contract .
You will be required to work an agreed number of sessions across the areas that the DDPCN HVS supports.
DDPCN works in partnership and provides the home visiting service (Monday to Friday) for the seven GP Practices within our locality namely:
We are a very supportive PCN &will support you to work across the different surgeries and will have a lead GP/Clinical Director.
This is a great opportunity to get toknow our practices working across the north ,central and south Derbyshire Dales areasand will provide the successful candidate with varied experience.
To perform as an autonomous general practitioner workingindependently and with other health care professionals to assess, diagnose and treat the conditions of patients that have been triaged at practice level andrequire an acute /same day home visit or a proactively assessed visit thatfalls outside of the urgent criteria.
To provide expert professional advice to patients, carersand colleagues and ensure the maintenance of clinical excellence. To beinvolved in the provision of education and training of other staff.
To be professionally accountable and responsible for allelements of clinical practice in accordance with the GMC code of conductensuring the safety of patients and quality of their care is primary.
The post holder will need to be capable of independenttravel across the Derbyshire Dales area and work flexibly to meet the needs ofthe Business.
Derbyshire Dales PCN is made up of 11 GP Practices as listed above .
There is an excellent road infrastructure and access to and from the M1, A6 and A38 is easily achievable .

Clinical responsibilities
In accordance with the Home Visiting Service (HVS ) rota, as agreed, the post-holderwill make him/her-self available to undertake a variety of duties includingconsultations, telephone consultations and queries, visiting patients at home,checking and signing/authorising repeat prescriptions and dealing with queries,paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other health careworkers within the organisation
Assessing the health care needs of patients withundifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signsof illness
In consultation with patients and in line with currentpractice disease management protocols, developing care plans for health
Providing counselling and health education
Admitting or discharging patients to and from the caseloadand referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes toagreed standards
Compiling and issuing computer-generated acute and repeatprescriptions
Prescribing in accordance with the practice prescribingformulary (or generically) whenever this is clinically appropriate
The post-holder will be required to visit patients in anemergency if clinically indicated.
The post-holder will undertake fair share of home visitingsessions per week as allocated.
You will be experienced and be competent using the clinical platforms of EMIS and Systm1.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
