Job Summary

The Borchardt Medical Centre is seeking a proactive, organised, and patient‑focused Care Coordinator to join our friendly General Practice team. This role supports patients in accessing the right care at the right time, improving continuity and quality of services across the practice and the wider Primary Care Network (PCN). As a key member of our multidisciplinary team, you will navigate health, social care and voluntary sector services, support long‑term condition management, and contribute to better patient experience and outcomes. The role also includes phlebotomy and clinical filing/coding.

Responsibilities
  • Act as a central point of contact for patients needing additional support.
  • Coordinate appointments, investigations and follow‑up actions.
  • Help patients understand and access a wide range of services.
  • Support patients to attend and engage with their healthcare appointments.
  • Identify patients who require reviews (frailty, chronic disease, cancer care).
  • Work with GPs, nurses, pharmacists, social prescribers and wider PCN colleagues.
  • Manage recall systems for long‑term condition clinics.
  • Maintain accurate records on clinical systems (EMIS).
  • Prepare information for MDT meetings and follow‑up actions.
  • Support quality improvement and patient experience projects.
  • Build trusting and supportive relationships with patients, carers and families.
  • Liaise with community teams, hospitals, social care and voluntary organisations.
  • Encourage self‑management and empowerment of patients.
  • Perform venepuncture safely and in line with practice protocols.
  • Correctly label, record and handle specimens.
  • Ensure patient comfort, identification and informed consent.
  • Recognise and escalade concerns or adverse reactions.
  • Support the management of the practice’s Single Point of Contact (SPOC) system for chronic disease and care‑home coordination.
  • Provide administrative support to clinicians and the Practice Manager.
  • Participate in mandatory training, induction, appraisal and service development activities.
  • Maintain a clean and effective working environment.
  • Undertake other reasonable duties aligned to the role.
Qualifications

Essential

  • Strong organisational and administrative skills.

  • Ability to work with vulnerable or complex patients.
  • Confident IT user (including EMIS).
  • Able to work independently and as part of a team.

Desirable

  • Experience in health, social care or community support roles.
  • Understanding of personalised care approaches.
  • Knowledge of local NHS and social care pathways.

Employment Details

Job Type: Part‑time

Hours: 16 hours spread over 3 days. A Friday is non‑negotiable (8–8:30 am), with flexibility for the other days.

Salary: £12.35–£12.77 per hour (probationary rate £12.35, wage review after successful completion of compulsory training and probation).

Contract: Permanent
