Venn Primary Care Network (PCN) is seeking a Fully Qualified Advanced Clinical Practitioner (ACP) and Independent Prescriber to join our established Home Visiting Team.

About the role
  • Visit housebound patients across our network of 9 GP practices, delivering high‑quality clinical care
  • A full UK driving licence and access to your own vehicle are essential, as the position involves travel across Hull and East Riding
Main duties of the job

After completing the necessary clinical supervision, the post‑holder will work as an independent practitioner, delivering high‑quality care to patients with undifferentiated and undiagnosed conditions. This includes:

  • Face‑to‑face assessment, examination, diagnosis, treatment, management, referral, review, or discharge, in line with the Competency Framework
  • Utilising a broad range of clinical skills and advanced knowledge to ensure safe and effective patient care
Key responsibilities
  • Plan, prioritise, and provide patient care at all stages of the patient journey
  • Communicate complex clinical information effectively to patients, carers and the multidisciplinary team
  • Provide health education, support and counselling, including breaking bad news and explaining treatment options
  • Ensure care pathway safety and manage escalation, including immediate referral to specialist services when necessary
  • Document all care accurately and contemporaneously in the patient administration system
  • Lead service development activities such as audits, surveys and research required for service improvement
Person specification
Qualifications – Essential
  • BSc level degree in Nursing or a relevant health subject, with professional registration
  • Clinical‑based Masters degree or equivalent experience
  • Fully qualified Advanced Clinical Practitioner and Independent Prescriber
  • Maintain NMC/HCPC registration and adhere to professional conduct standards
  • Post‑graduate university modules completed in minor injuries or illness
Qualifications – Desirable
  • Teaching or mentoring qualification
Driving and Own Car – Essential
  • Full driving licence and access to a personal vehicle required for travel across Hull & East Riding
Experience – Essential
  • Experience in a specialist environment, demonstrating effective leadership and autonomy
  • Experience of flexible workload management and meeting deadlines
  • Experience of multidisciplinary team working and managing patients with minor injuries and illnesses
  • Proficiency in software such as Patient Centre, SystmOne, Clinicom, E‑Mail, Excel, PowerPoint and Word
  • Experience of the initial management, stabilisation and onward referral of patients with serious or life‑threatening conditions
Experience – Desirable
  • Previous leadership and management within a clinical setting
  • Post‑registration experience of leading a staff team
