A leading mental health charity is seeking an Enhanced Care Recovery Practitioner to support individuals with multiple needs, facilitating their engagement with essential services. Location: 74 Kirkgate, Leeds LS2 7DJ
Working Hours: 37 hours, Monday-Friday
Contract Type: Permanent
Salary: £25,110 - £32,090 per annum

As an Enhanced Care team Recovery Coordinator, you will be employed by Waythrough to support higher need service users who are unable to progress their timeframe due to their multiple-needs. Multiple needs include but not limited to; chronic physical health, comorbidities, financial issues, memory / cognitive impairment, age, housing, limited social support and other multi-agency support needs.
