This is a key role in the Better Access team. You will beexpected to provide excellent practice nursing care directly to patients andsupport the development of innovative and high-quality nursing practice,offering advice on service improvements based upon your experience andknowledge of the needs of patients.

This post is offered as a "Bank"position with avariety of shifts available during working hours, evenings and weekends,offering you the flexibility of when you work.

We deliver Diabetes education programme, RespiratoryDiagnostics Service and our Enhanced Access Service. Rates of Pay - Enhanced Access (Evenings and Weekends)

Practice Nurse - £33.00 plus 12.07% annual leave allowance

Rates of Pay - In Hours (Between 8.00am - 6.00pm)

Practice Nurse - £22.75 plus 12.07% annual leave allowance

Main duties of the job

To work within the multidisciplinary team to meet commongoals.

To provide family planning advice

Monitoring of ongoing treatments.

Carry out long term condition reviews Diabetes, Asthma etcetc

Perform cervical cytology screening.

To provide opportunistic screening and advice on patientshealth.

To assist with planning and evaluating patient care.

Clinical measurements including blood pressure recording,blood glucose monitoring and ECGs

Undertake wound care and appropriate management of wounds.

To provide Health Promotion in areas such as weight, smokingand alcohol.

Teaching patients and their family self-care whereappropriate.

Assisting and or chaperoning the general practitioner.

To work consistently within your own professional standards,maintaining an awareness of evidence based and current best practice.

To ensure appropriate registration with your ownprofessional body.

Maintain cleanliness and hygiene to infection controlstandard

Heartbeat Primary Care Community Interest Company is the GP Federation for the three Primary Care Networks (PCNs) of Hambleton (North & South) and Richmondshire. Our shareholders are the GP Practices in these localities.

Heartbeat's mission is to improve health and care by supporting the communities we serve with innovative quality services. As well as providing support and opportunities for at-scale working to our member practices and PCNs, we are a healthcare provider delivering a range of General Practice services from multiple locations across the area, including for patients of our own list at Harewood Medical Practice.

We deliver various clinical services during the week, on evenings and weekends to patients in various locations across Hambleton, Richmondshire.

The Enhanced Access service operates on behalf of Hambleton and Richmondshire PCNs and is delivered from a number of local General Practices and hospital.

The role is to cover a variety of shifts which are convenient to you. We offer in hours shifts (14.00-18.00hrs), and shifts on an evening (18.30 - 20.00hrs) and weekends (Sat 09.00-17.00hrs & Sun 09.00-13.00hrs).

We have many clinical services which require the expertise of a Practice Nurse to complete the service delivery team.

We deliver Diabetes education programme, Respiratory Diagnostics Service and our Enhanced Access Service.

Clinical Tasks

  • Clinical measurements including blood pressure recording, blood glucose monitoring and ECGs
  • Perform cervical cytology screening.
  • Undertake wound care and appropriate management of wounds.
  • Undertake long term condition reviews and record via Ardens templates
  • Prepare, maintain, and restock treatment areas and equipment before and after patient appointments.
  • Participate in health promotion and patient education activities.
  • Maintain cleanliness and hygiene to infection control standards
  • Escalate clinical concerns to a senior nurse or GP promptly.

Communication

  • Communicate sensitively and effectively with patients, carers, and team members.
  • Adapt communication methods to suit patients' individual needs, backgrounds, and understanding and perspectives.
  • Maintain open, respectful, and professional communication within the team and with external partners.
  • Represent patients needs and perspectives when appropriate.
