Digital & Transformation Lead - Operations

This role is for 30 - 37.5 hours per week.
As the Digital and Transformation Lead for Operations, you will support improved patient access to care through the effective setup, oversight, and continuous improvement of services. You will contribute to the management of staff, demonstrating a sound understanding of HR processes and procedures, while ensuring contractual compliance and financial sustainability. You will provide clear and timely oversight of services to the PCN Board, ensuring members are appropriately informed to enable effective and well‑evidenced decision‑making.
You will work across 8 practices within Greater Middlesbrough, providing Operational Management support to PCN staff and will be responsible to the PM Working Group.
Main duties of the job
- Support the management of PCN staff, ensuring equitable workload distribution across member practices.
- Ensure contractual compliance and contribute to the financial sustainability of the PCN.
- Lead the adoption and optimisation of digital solutions to improve patient access, experience, and PCN productivity.
- Build effective relationships across practices and the wider system to support collaborative delivery of care, including enhanced access models.
- Lead and support continuous improvement and quality improvement approaches, including clinically led innovation and integrated working at neighbourhood or place level.
- Support the PM Working Group and DES Leads in the implementation of new initiatives and programmes.
- Lead and deliver projects that improve service delivery and health outcomes for the PCN population.
- Apply best‑practice project and change management methodologies to ensure delivery, benefits realisation, and successful adoption of initiatives.
- Support organisational change and provide flexibility to support new and emerging programmes.
- Extract and analyse contract and performance data to support delivery and reporting.
- Maintain robust project documentation and reporting systems, including dashboards, highlight reports, and risk and issue logs.
- Analyse complex information to produce accurate project reporting and support informed decision‑making.
- Champion best practice, learning, and innovation across the PCN to support continuous improvement.
- Prepare and present regular progress reports to the PM Working Group and PCN Board, including risks, issues, dependencies, and recommendations.
- Communicate and engage effectively with PCN leadership, member practices, and stakeholders to support strategy development and programme delivery.

Person Specification
Experience
- Experience of supporting or managing staff across multiple teams or organisations, ensuring equitable and effective workload distribution.
- Knowledge of PCN DES requirements and NHS contractual frameworks.
- Experience of managing staff and HR processes.
- Demonstrable experience of ensuring contractual compliance and supporting financial sustainability within a healthcare or public sector setting.
- High level of organisational skills, including maintaining robust project documentation, dashboards, and risk/issue logs.
- Ability to adapt to change and support new and emerging programmes as service needs evolve.
- Experience of working collaboratively across multiple stakeholders, organisations, or practices to deliver shared objectives.
- Experience of leading or supporting service improvement, quality improvement, or change initiatives.
- Experience of analysing performance, contract, or activity data to inform delivery and decision‑making.
- Experience of working within a Primary Care Network, GP practice, or wider NHS system.
- Experience of supporting clinically led innovation or integrated working at neighbourhood, place, or system level.
- Experience of enhanced access models or shared workforce/appointment delivery across practices
- Experience of project delivery, including planning, implementation, monitoring, and benefits realisation.
