A leading healthcare provider in London is seeking an experienced Specialist Nurse to join their Diabetes and Endocrine team. You will provide expert nursing care in inpatient and outpatient settings, managing complex diabetes treatments and participating in multidisciplinary clinics. The ideal candidate should hold a relevant nursing degree, have experience in diabetes care, and possess strong communication skills. This fixed-term full-time position offers a salary range of £47,810 to £54,710 per year.
We are looking for an experienced band 7 Specialist Nurse to join our Diabetes and Endocrine team, to provide excellent inpatient/outpatient diabetes care cross site and cover the Virtual wards. The post holder will provide clinical expertise both in inpatient and outpatient care, and will join a dynamic team based at WMUH and Chelsea & Westminster Hospital.
The Diabetes Nurse Specialist provides expertise in all aspects of diabetes care provision. This would involve delivering excellent diabetes care to individuals and patient groups as a component of the patients' care management, in collaboration with the wider multi-professional healthcare team, as well as being a provider of excellent nurse teaching and ward education.

Working autonomously whilst part of a multi-disciplinary diabetes team, assessing and managing a caseload of diabetes patients and maintaining effective communication with patients, carers and professionals to ensure seamless service delivery.
Provide expert knowledge of diabetes and work within an agreed job plan to meet service needs, including knowledge and competency in insulin pumps and pump therapy, insulin administration, all diabetes treatments, and monitoring devices including continuous glucose monitors.
Undertake nurse-led diabetes clinics, conducting and interpreting relevant tests and investigations.
Work collaboratively with the multi-professional diabetes team, supporting medical review clinics, contributing expert, evidence-based diabetes knowledge and skills, including participation in obstetric/diabetes antenatal clinics for pregnant women with pre-existing or gestational diabetes, and in consultant-led insulin pump and medical review clinics.
Maintain boundaries of own practice within guidelines and use professional judgment in managing complex situations.
Address patients\' risk factors and health promotion needs through comprehensive assessment to reduce diabetes-related complications.
Establish an environment of trust and empowerment in diabetes care, acting as an advocate for patients in treatment decisions and choices.
Utilise multi-agency and inter-professional resources to ensure patients receive appropriate services, making referrals as needed.
Provide excellent and effective communication to convey information, explanations and reassurance in a sensitive, empathetic manner.
