An excellent opportunity has arisen to join a progressive, patient-focused GP practice committed to delivering high-quality primary care services for Practice nurse who specialises in Diabetes. The ideal candidate will have a strong background in general nursing, with specialised skills and knowledge in diabetes care. This role involves providing high-quality nursing care, managing and supporting patients with diabetes, and working collaboiratively with other healthcare professionals to ensure the best outcomes for our patients.

Main duties of the job

  • Provide comprehensive nursing care to patients with acute and chronic health conditions.
  • Conduct thorough assessments of patients with diabetes, including monitoring blood glucose levels, HbA1c, and other relevant health indicators.
  • Develop, implement, and evaluate individualized care plans for patients with diabetes.
  • Offer education and support to patients and their families about diabetes management, including diet, exercise, medication, and lifestyle modifications.
  • Perform routine health checks and manage any complications related to diabetes.
  • Clinical Duties:
  • Review diabetic patients and optimise their care plans
  • Insulin initiation and titration would be desired though not essential
  • Collaborate with other healthcare providers to coordinate comprehensive care for patients with complex needs.
  • Participate in chronic disease management clinics and group education sessions.
  • Provide personalised education and counselling to patients on diabetes self-management.
  • Develop and distribute educational materials and resources to enhance patient understanding and compliance.
About us

Cotswold Medical Practice comprises of two surgeries, one atMoore Health Centre in Bourton in the Water and the other at Westwoods surgeryin Northleach.

Our clinical team consists of eight GP Partners, twosalaried GPs, a Nurse Manager, four Practice Nurses, three HCAs andphlebotomist.

Person Specification
Qualifications
  • Registered Nurse (RN) qualification with current NMC registration.
  • Minimum of 3 years of experience in general practice or primary care nursing.
  • Specialised training or certification in diabetes care (e.g., diabetes education, diabetes management).
  • Strong clinical skills and ability to manage complex cases.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficient in the use of electronic health records and other healthcare technology
