Qualifications

• Minimum of 6 months recent care delivery experience in a health or social care setting.
• Strong communication skills and ability to work within a multidisciplinary team.
• Knowledge of holistic care planning and symptom management.

Responsibilities

• Deliver holistic, person-centred end-of-life care under guidance.
• Assist with daily living activities and contribute to care planning.
