Overview

Lincolnshire Community Health Services NHS Trust is home to a community nursing team that is dedicated to delivering person-centred care in patients’ homes, care homes and clinic settings. The post is available through a flexible 12‑week induction programme that is supported by robust preceptorship and clinical skills provision.

Responsibilities
  • Work autonomously and as part of a team to deliver nursing care to patients in their own homes, in care homes and in clinic settings.
  • Travel to patients’ homes and clinic settings – access to personal transport is essential due to rurality of Lincolnshire.
  • Deliver care during variable shift patterns: early shifts 8:00 am – 4:00 pm, 9:00 am – 5:00 pm and late shifts 10:30 am – 6:30 pm, 7 days a week.
  • Use evidence‑based practice to assess care needs, plan, deliver and evaluate nursing care.
  • Participate in the 12‑week induction programme, a robust preceptorship and clinical skills programme and peer support initiatives.
Qualifications and Experience
  • Registered Nurse (RN) with a valid nursing registration.
  • Valid DBS check – new employees will be subject to a DBS check fee deducted over a maximum of six months; existing employees may also be charged if required.
  • Excellent communication skills and the ability to document nursing care accurately.
  • Good organisational and judgment skills, with the ability to prioritise and manage time effectively while working independently, and without significant support.
