Job Summary

We are seeking an experienced and enthusiastic Band 6 Community Psychiatric Nurse to work within community services in the borough of Merton for South West London and St George's NHS Trust, within the South West Merton Integrated Recovery Hub. This 9‑month secondment post offers a chance to work as an integrated member of a dedicated community team, working with adults experiencing mental health issues. Our community services are innovative and have recently been reconfigured to provide specific recovery‑focused interventions to people with mental health issues accessing secondary care. The focus is on providing time‑limited interventions to enable individuals to maintain their lives with limited intervention; to understand and develop strategies to manage symptoms they may be experiencing, while working in partnership to establish their own personal recovery action plans.

Job Responsibilities
  • Manage a defined caseload designed to meet the individual needs of the client and carers.
  • Maintain a high standard of individualised client care, recognising factors and events that may affect the mental, emotional and physical well‑being of clients/carers and provide advice and support when required.
  • Act as a care coordinator, assessing clients’ needs and strengths, ensuring all clients have care plans that are implemented and reviewed/updated.
  • Develop and maintain a high standard of practice based on assessment, using psychosocial approaches within a health and psycho‑social framework.
  • Communicate and liaise effectively within relevant agencies to ensure an integrated programme of care.
  • Implement or participate in crisis intervention measures with the client/family or in conjunction with other agencies.
  • Supervise and monitor therapeutic effects and side‑effects of prescribed medication, providing clients with clear advice and information in line with Trust policy and NMC standards for drug administration.
  • Organise and participate in the admission, discharge and transfer of clients in accordance with Trust policies.
  • Provide clients and carers with verbal and written up‑to‑date information on the services offered, how to express views, complaints procedure and what they can expect.
  • Attend weekly clinical meetings, providing the team with relevant information on caseload, referrals and feedback on new assessments.
  • Develop and maintain effective working relationships with inpatient facilities regarding admission and discharge.
  • Facilitate carers’ assessments, provide appropriate care plans to meet identified needs and monitor efficacy.
  • Have knowledge and application of key legislation and policy such as the Mental Health Act, Care Programme Approach, Capacity Act and Care Act.
  • Participate in the running of clozapine and depot clinics.
Person Specification
Essential Knowledge & Skills
  • Knowledge and use of relevant NICE guidelines and evidence‑based interventions.
  • Knowledge of counselling, group, cognitive and behavioural skills.
  • Experience of clinical assessment, risk assessment, MSE, mental capacity assessment and crisis management.
  • Good verbal and written communication and report‑writing skills.
  • Knowledge of current government policy and relevant legislation concerning community mental health care.
Desirable Knowledge & Skills
  • Brief Solution‑Focused Therapy.
  • Coaching.
Essential Experience
  • Providing thorough baseline mental health assessments and case formulation based on these.
  • Working in a community multidisciplinary team, undertaking home visits and attending various clinical sites as required.
  • Working with service users presenting with a range of mental illnesses, including those in crisis.

  • Managing and supervising other staff.
