Are you an experienced, compassionate nurse ready to take the next step in your career? We are looking for a motivated Community Nurse Team Leader to join our dedicated team at St Martin's Hospital, Bath, supporting individuals across Bath and North East Somerset. In this rewarding community-based position, you'll provide high-quality nursing care, advice and support to housebound patients in their own homes and care homes—helping them maintain independence, enhance well-being, and avoid unnecessary hospital admissions.

Responsibilities
  • Deliver expert clinical care to patients in their own homes
  • Provide strong leadership and day-to-day management to a small team of Community Staff Nurses and Healthcare Assistants
  • Act as the clinical lead for the caseload, ensuring all patients receive appropriate assessments and personalised treatment plans
  • Work independently with minimal supervision while collaborating closely with the wider multidisciplinary team
  • Uphold all clinical policies, procedures and best‑practice guidelines
