If you are passionate about providing high quality individualised patient care, enthusiastic about supporting patients' rehabilitation and increasing their independence and quality of life we would like to hear from you.
You will provide holistic nursing assessment, high quality care, support, advice, and information to patients and carers with the aim of maintaining patients in their own homes, enabling them to enjoy maximum independence and optimum quality of life.
Ensure the effective co‑ordination and provision of the nursing service in the absence of the Zone Nurse Lead and to actively participate in the planning and discharge of complex patients from secondary to primary care.
Working across the Intermediate care and community nursing teams to provide crisis response nursing interventions, assessment, liaison, referral, co‑ordination, delegation of care, or placement in order to prevent hospital admissions. Role requires driving licence as community based.
Torquay community nursing are a large but supportive team who enjoys mentoring team members and students and is passionate about delivering quality care to the housebound patients.
