Qualifications

• Registered Nurse with current NMC registration.
• Full UK driving licence and ability to use a car.
• Knowledge of recent NHS changes and political drivers for community care.
• Strong communication, interpersonal, and organizational skills.
• Ability to work independently and innovate to meet service need.
• Clinical competence in wound care, medication administration, intravenous lines, venepuncture, and catheter care.
• Willingness to perform travel across CCICP footprint.

Responsibilities

• Assess, plan, implement, and evaluate patient care.
• Provide skilled nursing care including wound care and medication administration.
• Teach and supervise nursing care that can be delegated to patients/carers.
• Record all patient contacts accurately.
• Manage and assess patients' discharge potential.
