A community healthcare provider in Shefford is seeking a motivated Band 5 Community Nurse to join their Mid Bedfordshire Primary Home Care Team. The role focuses on high-quality nursing for patients with complex needs, facilitating self-care, and promoting health. We are seeking a motivated, dynamic, and passionate Band 5 Community Nurse to work within our Mid Bedfordshire Primary Home Care Team, the team deliver responsive high quality nursing to patients living in Shefford and surrounding villages within Mid Beds. We are passionate about patient centred care, support our nurses to lead and deliver excellent and compassionate nursing and value our clinical teams as the real heart of the NHS. You will work closely with other members of the MDT to deliver care to patients with complex needs.
This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams.
