Community Nurse

An exciting opportunity has come up to join our busy Neighbourhood Team. We are looking for an enthusiastic, hardworking, Band5 Community Nurse to work 30.0 hours per week. Community Nursing is an integral part of the NHS 5-year plan with an emphasis now being placed on admission prevention.

You will need to be passionate about community services and shaping a new way of delivering care.

We are an integrated team who work collaboratively and compassionately with therapists, local authority services, the urgent care response team and our colleagues from The Primary Care Network to provide patient‑centred, responsive care which meets the complex and varying needs of our housebound community.

Main duties of the job
  • The ability to communicate effectively at all levels
  • Flexible working to provide care across the local area (the service operates from 7:30 am – 6:30 pm, Monday – Sunday incl. bank holidays)
  • An enthusiastic, motivated and innovative thinker
  • The ability to work autonomously, manage your own caseload and work within a multi‑disciplinary team
  • The ability to problem‑solve
  • Experience of working in a fast‑paced and varied environment
  • Perform home visits for adults over 18 years needing a range of nursing interventions, including end‑of‑life care, wound care, IV therapy, PICC line care and catheter care
  • Responsible for holistic assessments of patients taking into consideration their environment and social circumstances
  • Support the education of student nurses, both pre‑ and post‑registration, as well as nurse associates and health‑care assistants
  • Oversee patients and assist in decision‑making regarding their care as a band 5 nurse
  • Report to immediate Band 6‑line manager and receive support from the wider leadership team; attend daily handovers and participate in multi‑disciplinary team meetings

Qualifications
  • Registered Nurse (RN1)

  • Accredited courses in palliative care, tissue viability, diabetes
  • Practice Assessor training
