Job description

Could you be part of an innovative, caring and agile Community Nursing team? Do you want to work as part of an award‑winning team? Welwyn and Hatfield Integrated Care Team are winners of Hertfordshire Community Trust Leading Lights awards for Demonstrating Fantastic Patient Care and are looking to recruit band 5 Nurses to our award‑winning team. Hertfordshire Community Trust has a Good CQC rating acquired in May 2020 and we are working towards achieving an Outstanding rating.

Role and Responsibilities
  • Deliver high quality nursing care across the Welwyn and Hatfield locality to prevent admission to hospital, facilitate discharge, deliver planned nursing intervention and manage long‑term conditions.
  • Support skill development and join a team committed to demonstrating the Trust values that care about each other as well as their patients.
  • Work collaboratively with partnership organisations across East and North ICB, Hertfordshire Foundation Trust and Hertfordshire.
Qualifications
  • Must be a band 5 nurse.
  • Ability to creatively problem‑solve and work on your own initiative.
  • Demonstrated understanding of current professional and community issues.
  • Community nursing experience preferred; training and development opportunities available for suitable applicants.
  • Full valid driving licence and access to a car for business purposes (unless you have a disability as defined by the Equality Act 2010).
