A regional NHS healthcare provider is seeking a motivated practitioner to join the Older Adult Community Mental Health Team. This role focuses on advocating for older adults with complex mental health needs in the Preston Central, Chorley, and South Ribble areas. The successful applicant will deliver evidence-based interventions and collaborate with families and multidisciplinary teams to promote recovery-based care. Flexible working arrangements are available to promote work/life balance.
The Older Adult Community Mental Health Team (OACMHT) would like to welcome a highly motivated practitioner who is passionate about advocating for Older Adults. The practitioner would be expected to be adaptable as well as creative and have the ability to work autonomously and as part of the multi‑disciplinary team. The team will provide intervention based care to service users who present with complex mental health needs and cover the Preston Central, Chorley and South Ribble area.
This post is based at Charnley Fold, Bamber Bridge and requires Monday to Friday working. Flexible working can be discussed prior to appointment to suit the service needs and employee preferences.

The successful applicant will have the ability to work with service users who have severe and enduring mental health issues, significant functional or psychological needs, dementia and comorbid physical health issues. Responsibilities include working with families and carers alongside other agencies to promote well‑being and deliver evidence‑based interventions, fostering recovery based care, and working alongside the community transformation.
Applicants will need to demonstrate competency and experience in line with the person specification and be willing to develop their skills. The successful applicant will be open to new ways of working, fostering and promoting recovery based care and working alongside the community transformation.
