A healthcare provider in Tees Valley is seeking a motivated Health Care Assistant to support patients in community settings. Candidates must have NVQ level 3 in health and social care and strong communication skills. This part-time role offers a salary ranging from £24,937 to £26,598 per annum, with various shift patterns, including nights and weekends.
We are inviting applications for a highly motivated and dynamic Health Care Assistant to join North Tees & Hartlepool Homefirst Response Service. This service will provide care to patients within a range of settings including patients own homes and residential care homes.

The post involves liaising with a variety of health and social care providers and other agencies to ensure the provision of high quality care management to this group of patients and their carers.

We require a Health Care Assistant with NVQ level 3 in health and social care who has excellent communication skills and is confident working across professional boundaries. You will need to be flexible, adaptable & a good team player with the ability to work independently within a community setting. You will be prepared to work all shift patterns within the locality including early, late and night shifts, weekends and bank holidays. You will be expected to have a full UK driving license and access to a car. You will be prepared to drive to patients within the whole locality area.
