Job summary

A healthcare organization based in Teddington is seeking enthusiastic Community Healthcare Support Workers to provide urgent care in the community. The role involves supporting patients under the direction of a registered nurse and requires a Level 3 qualification in Health and Social Care. Community HCA (Urgent Community Response)

The closing date is 24 December 2025

We are looking for enthusiastic Community Healthcare Support Workers for RRRT to work in our Urgent Community Response pathway. This role is to provide care and support for patients in the community setting as delegated by the registered nurse.

Access to a motor vehicle is essential, with appropriate driving licence.

Please ensure that your application demonstrates that you meet the essential criteria as listed in the job specification. Experience of working in urgent community response is beneficial.

The team operates from 08:00-20:00 7 days a week.

Main duties of the job

To work within RRRT to provide care and support for patients on the urgent community response pathway as delegated by the registered nurse.

You will be expected to carry out all relevant forms of nursing care, without direct supervision, and may be required to demonstrate simple procedures to qualified and/or unqualified staff undertaking training where agreed with the nursing team.

Person Specification
Experience
  • Functional Skills in Maths and English (Level 2) or equivalent
  • Level 3 qualification in Health and Social Care (Or equivalent experience)
  • Experience supporting urgent response or rapid assessment services
  • Experience working in a community or domiciliary setting
  • Certificate of competency in venepuncture, ECG and NEWS2 (or willing to work towards)
  • Evidence of continued learning or development relevant to clinical support role
Skills
  • Knowledge of care and related procedures/ clinical observations
  • IT litEte with ability to use databases and other software packages (specifically Systm 1)
  • Basic clinical skills e.g blood pressure, temperature, pulse, urinalysis, blood glucose monitoring
  • Able to recognise (or learn to) and elevate patient deterioration or safeguarding concerns
  • Able to travel independently to patients' homes across the borough
  • Awareness of urgent community response services and their purpose
  • Understanding of the impact of Mental Health e.g. dementia, depression, on the management of medical and long term conditions
