Overview

We are inviting applications for a highly motivated and dynamic Health Care Assistant to join North Tees & Hartlepool Community Nursing Service. This service will provide care to patients within a range of settings including patients' own homes and residential care homes.

Responsibilities
  • Provide a whole person approach to the care and management for patients within a community setting, working autonomously.
  • Liaise with health and social care providers and other agencies to ensure provision of high quality care management to patients and their carers.
Requirements
  • Health Care Assistant with NVQ level 3 in health and social care.
  • Excellent communication skills and confidence working across professional boundaries.
  • Flexible, adaptable and a good team player with the ability to work independently within a community setting.
  • Willing to work all shift patterns within the locality including early and late shifts, weekends and bank holidays; may provide cover for out of hours District Nursing team.
Additional Information

The post holder will have a high level of knowledge in community care with interest in areas such as palliative care, long term conditions, catheterisation and wound care.

The post involves liaising with a variety of health and social care providers and other agencies to ensure high quality care management.
